It is the applicant's responsibility to provide all required information. If the IU Health Central Verification Office does not receive all documentation required for the Initial application within thirty (30) days from the initiation of the Initial application, the entire application will be marked as incomplete, and you will be required to start the IU Health process over.

All information for Reappointment applications should be submitted within 30 days to ensure the entire reappointment process is completed prior to the reappointment date.
